Union Supported Living works in partnership with social workers, commissioners and local authorities to provide safe, person-centred supported living services for adults with autism and learning disabilities.

Referral Process

Step 2 – Placement Review

Our management team reviews the referral information to determine whether we can safely and effectively meet the individual’s needs.

Where appropriate, we may arrange a meeting or discussion with the referring professional.

Step 3 – Assessment and Planning

If the referral progresses, we work collaboratively to develop a personalised support plan.

This may involve:

• meeting the individual
• reviewing goals and preferences
• assessing environmental and support needs
• planning staffing and support structures

Step 4 – Placement Implementation

Once arrangements are agreed, we work with all parties involved to ensure a smooth transition into supported living.
