Union Supported Living offers a range of support options depending on assessed needs.

• supported living placements

• ongoing outcome monitoring and review

Support levels can be adapted over time as needs change.

Referral Process

We aim to make the referral process straightforward and responsive.

Step 1 – Initial Enquiry

Contact our team to discuss the individual’s needs and placement requirements.

We may request initial information such as:

• care assessments
• support plans
• risk assessments
• behavioural support plans (if applicable)

Step 2 – Placement Review

Our management team reviews the referral information to determine whether we can safely and effectively meet the individual’s needs.

Where appropriate, we may arrange a meeting or discussion with the referring professional.

Step 3 – Assessment and Planning

If the referral progresses, we work collaboratively to develop a personalised support plan.

This may involve:

• meeting the individual
• reviewing goals and preferences
• assessing environmental and support needs
• planning staffing and support structures

Step 4 – Placement Implementation

Once arrangements are agreed, we work with all parties involved to ensure a smooth transition into supported living.

Our aim is to provide stability, consistency and ongoing communication.

Safeguarding and Compliance

Union Supported Living is committed to delivering safe, accountable services.

Our safeguarding practices follow the principles outlined in the Care Act 2014.

Staff are trained in safeguarding, autism awareness and person-centred support.

Where applicable, our services operate in line with standards set by the Care Quality Commission.
